Hey guys, slightly gross/medical question... I've got a tear near my asshole that for several years has with some regularity opened up during a number 2, and especially during sex (while bottoming). It's very painful when this happens and is totally disruptive to my sex life in a way that makes me feel crazy.

I take a fiber supplement, drink lots of water, am fit, etc. I've done courses of treatment with that ointment prescribed for treating this kind of thing with success... but once healed, it still always eventually comes back.

Saw a proctologist about it and he advised that it's simply the kind of thing that will come back potentially forever and requires management. But... I'm wondering whether this is true?? He and other doctors I've seen about it seem to all be straight men who I'm guessing aren't getting fucked regularly, and I can feel that they don't really grasp what a significant thing is for this problem to be ongoing.

I'm wondering whether anyone has any tips, insights, or approaches to dealing with this problem?

It's not my specialty so I don't know anyone but may I suggest a different starting point and go along to a sexual health clinic such as the John Hunter Clinic at Chelsea & Westminster Hospital. They will at least have staff who understand the scale of your problem and if anywhere can give you pointers about where to go next, they will be able to.

I'll keep thinking and if I come up with any answers I'll post them to you. Part of me thinks this ought to be easily mendable with a couple of stitches after an enema and a thorough rectal exam just to make sure there's nothing hiding or more complex going on. You would need to lay off sex for about a month afterwards but in the scale of life, that's nothing.

thanks very much for this!! very helpful xx

I think there are two things that might help: whenever you poop, use a moist toilette not dry paper, and consider applying lotion after each cleaning. Second... always use a lot of lube for anal sex. The tissue inside as on the entry is the thinnest most delicate tissue in your body. And find tops who will lube your hold up with a finger before anything is else is inserted in you. Seriously. LUBE LUBE LUBE. And i recommend silicone based because it doesn't wash away easily. Water based lube can dry up in a minute and defeat its intended purpose -- to protect your skin.

The Asshole is not self-lubricating like the Mouthhole or Pussyhole.

I am not a physician but it is my understanding that sometimes these fissures persist due to lack of blodd circulation because the sphincter tightens due to the pain. From that perspective, Botox injectinion have been used with some success. Fifty years ago, or so, a standard treatment was mechanical dilation with dilators . I actually have no idea how effective that was or how helpful that approach would be to someone who bottoms with some regularity. Here in the U.S. Amazon sells a set of dilators that are graduated in diameters so one can "work up." Good luck to you in getting this vexatious problem resolved.

Firstly stop enjoying anal penetration and stimulation. Next take a simple thing like Liquid Parrafin to soften up your crap, shiting soft is really good even if messy.
If it still persists seek medical help, that way you can be repaired and not be at risk of infection.

I had been suffering from something like this for a few years. I tried all of the above things mentioned including Botox and surgery which definitely helped, but what got me back to feeling 100% down there was using an Aneros device for long periods with a Vaseline type lube. Part of the issue with healing down there is the tightening of muscles over time in response to the pain and discomfort and the restricted blood flow. The Aneros device helped restore blood flow down there and it felt like my tears just melted away in a pretty quick period of time. It was a long time coming but I was able to get fucked again after 5 years of failed attempts. Has been great for state of mind as well. I couldn’t recommend anything more. Good luck.

Interesting. How long are long periods of time?

Which Aneros product do you use if you don't mind me asking?

I have a couple of their devices. But for this purpose specifically I would suggest starting with the peridise set of 2, as they are less girthy than others. They resemble a device designed for hemorrhoid treatment in a similar fashion (can’t remember the name). If you get into the use of them generally then I’d recommend the tempo too! haha. Happy to chat with you or anyone in more detail if I can help out.

There's this hole cream by Asset.. after multiple failed procedures and Botox that's the only thing that worked!!

You may have a persistent fistula. Check out Dr. Evan Goldstein's website in the US. Bespoke Surgical. He is a gay colon rectal surgeon. He may have some tips and tricks on his website.
