The device appears to be a cleverly-designed cock-ring consisting of an inflatable torus (doughnut) constrained in all directions except its inner diameter. Presumably the device is placed at the base of the penis, pushed as close as possible toward the pubic bone, and inflated. This will trap blood in the penis like any cock ring. I like that it can be set very tight, without worry about getting it off later (as with a metal ring). I've thought about either getting one (or building one) to use in conjunction with pumping. This would seem a perfect combination for girth expansion.
If placed at the base of the glans (head), the device could also be used as an attachment point for length stretching. Attaching to the end of the penis always seems the problematic area for lengthening devices and get quite complicated IMHO.
