The procedure to get artificial penis constructed is pretty hardcore. I'll have the details for some other day and just say that it's intense and cuts you up in various places other than down there to get it together to even resemble a penis. Here area few links of the finished products.

No you don't get to pick the size they seem to air on the side of a bigger penis though! But it's all down to the donor site and how much fat you have as the skin graft it lifted with the fat under it and then made into a penis
Some surgeons offer a choice or shorter or longer some just say you will get whatever size they make
Depends really, if there is an erectile pump installed to induce erection etc. They take the sensitive areas like the clit and what not (don't spend much time down there lol sorry) and they use that to supply sensation to the tip (USUALLY). Not all genital reconstructions are created equal.
Can they come in an uncircumcised model? All of these pictured seem to be cut. it looks like most women becoming male would want to be as much male as possible. Not to say that cut men are not all male.

Because in the states that is the common dick model. Cut is basically normal and they want that look of normalcy. Based on what I know about this surgery in particular and surgery in general... based on technology and technique it would difficult if not impossible and overly expensive.
it's crude, but I've heard, "anyone can make a women, but only God can make a man." Science is doing its best, but manufacturing a penis "uncircumcised" seems nearly impossible.
I took a class on human sexuality. The topic of sex changes was an interesting section, I believe the term our guest speaker used was "its easier to dig a hole than it is to build a pole"
