A friend of mine has a straight friend, ended up hooking with this girl. When the straight friend wanted to broke it off, the girl told him that she is pregnant. This straight friend decided to go to the abortion route, because this straight friend actually has a family and he does not want a string attached. This girl agreed. They decided to do it overseas, and in order to get her out of the country, she needed to process visa. And of course, this girl has to give her passport and documents. Then the surprise. She is not a cis woman, because the gender is actually write as male - not female.

That's the background of why I am asking the question.

The straight friend claimed he has no idea that she is not a cis woman. He did not feel any different - when having sex with her.

So now, I asked the straight guys here who also have experience with trans, is there really no difference? Like you cannot differentiate a trans (post op) woman over cis woman when having sex with her? Like in this case, to the point that he believed that she is pregnant?

Never got involved with a not cis woman, the odds is against your question. Anyway on a working environment I met a flew post op men and women and you can notice the difference obviously no sex involved.
Looking old pictures is usual to a couple and id documents ones are the closest and it's just there the gender, also paying with credit card you use an id.
I don't buy your friend story but assuming it was true, that woman it's a pathologic lier, what she wanted to do? get a vacation with him by deception? con him with a fake abortion to steal from him? All that knowing this men had another relationship? He did cheat but she knows that he has another life, if he spend 98% of the time away from her.

The first thing to do when you suspect pregnancy is to go to her gynecologist this woman won't pass the blood work the eco or the inspection of the doctor. Did she tampered a pregnancy test with markers and pee on it?
