It doesn’t cause discomfort as far as the skin. Use lube or a moisturizer during pumping.

I haven't done pumping for a while but I was a decades-long pumper and there are areas of my penis that will "balloon" up if I go too long. (3 hours +)

If I were you I would use a compression sleeve. I would pull the foreskin down and put the sleeve Overtop of it and pump at low pressure so you're head could get bigger and I would think that you could still get the satisfying glide of your foreskin maybe.
