Well, fwiw, I occasionally get a hair stuck under my foreskin and it really can be surprisingly painful to try to simply pull it out. But, my foreskin is fully and easily retractable, so it's no big deal getting anything out from under it.

See, if you had a national system of medical care that's free at the point of need, you'd just make an appointment to go see the doctor or nurse and get some professional advice.

If it fully retracts during sex (why do you 'believe' that it does) then it should do so when fully aroused.
If it doesn't then it needs seeing to by a professional - which does mean full circumcision - because other 'things' could be getting trapped under there ...... basic hygiene for any uncircumcised man is to retract his foreskin and wash, at least twice a day.
I don't know that it necessarily means full circumcision (not that there's anything wrong with that), but I agree that it should be dealt with.
