That depends what you mean by safe. In the short term, nothing bad will happen. Someone mentioned blue balls, but that isn't a threat to your health. Nothing will burst, though you may end up having a wet dream.

Longer term, a link has been found between ejaculation frequency and prostate cancer risk, i.e. you you seldom ejaculation your risk is higher than if you ejaculate often. At least 21 times/month was a suggested frequency.

But that doesn't mean that frequent ejaculation guarantees that you won't get prostate cancer or that infrequent ejaculation guarantees that you will.

Well, in the short term there is a considerable chance of contracting aspecific prostatitis, and if you abstain a bit longer, there is a very real chance of epididymal blow-outs (surprisingly common and may potentially negatively impact your sperm production). Long-term prostatitis can lead to cyst-like encapsulations in the prostate that can drive hyperplasia and also cause complaints when ejaculating.

That is actually not true. Also, long-term abstinence has been found to correlate to antibodies against one’s own sperm. And it can also lead to depressive episodes (probably due to lowered T)

Can you provide a reference for that in connection with voluntary abstinence? Searching it, I got lots of references to that happening after a closed vasectomy, but isn't that a rather different situation? Likewise, the papers dealing with the immune system being activated by one's own sperm were also connected with vasectomies and this was being studied in connection with the likely success of vasectomy reversal.

When the vas deferens has had the cut end closed off, yes of course there will be an increase in pressure before that closed end. That also seems to link into the immune system in that it seems the expansion of the closed vas deferens and epididymis continues until the stretching causes some minor damage to the inside surface. At that point a chemical marker attracts an immune response, i.e. a response that is there to deal with damage from pathogens, or damage to the skin that would let pathogens in. Up until then the immune cells have been kept away from the sperm as the sperm, being haploid, would not be recognised as native cells and would be destroyed, which is exactly how that increased pressure is then resolved.

In the case of voluntary abstinence, bear in mind I am not a urologist or even a doctor, but I have never seen any sphincters shown anywhere on the vas deferens or ejaculatory duct on anatomical diagrams. I can be very short about it: I’ve witnessed blow-outs, spermatocoele, as well as aspecific prostatitis in much, much greater numbers than expected in (post-)adolescents and young adults who all fell victim to that current ‘no fap’/‘semen retention’ craze.

As for the increased pressure after vasectomy, that’s actually a much greater problem than most will have you believe. I’ve come to the point of advising against vasectomies as the chance of long-term problems is considerable.
