The John Thomas sign, also known as the Throckmorton sign, is a slang or joke term used in the field of radiology. It refers to the position of a penis as it relates to pathology on an x-ray of a pelvis. When the penis (visible on the x-ray as a shadow) points towards the same side as a unilateral medical condition such as a broken bone, this is considered a "positive John Thomas sign", and if the shadow points to the other side, it is a "negative John Thomas sign."

So if those are fake or not, I dunno, but it seems that one's willy can be seen in an xray image.
 
Yes, they can be real. I took my grandmother to her orthopedic appointment. Her dr. had several xrays up on a light board. They showed a reconstruction that he had done to this poor guy who had been in some kind of accident. And yes, his dick was clearly visible along with several different types of hardware the Dr. had installed to put this fellow back together.
 
cool! very intimidating if they talk about your upcoming hip surgery and your dick is visible in front of everyone all the time haha...
just the other day i saw some real x-rays of a pelvis - and another dick appeared there as well
 
I have had many xrays done of my low back and i have never seen anything other than bone. Not once did i ever see anything that looked like a cock.
 
I have a young friend that had to have hip replacement surgery due to a bad car accident. He told me that when he went back for his check up they did an x-ray and his dick was clearly visible. It bothered him that his mom was there too when they showed him the x-ray.
