Wait what? Do people genuinely not know she’s a character Renato created? I don’t speak Brazilian, so I’ve never been able to understand what her character says/does, but it’s never seemed like it was hidden that it was him…?

In Brazil they speak Portuguese.

Most people get it as obvious, but he does a good job of keeping up the comical pretense - e.g. having ‘Karen’ comment on Renato’s posts in character, etc. - and of course the internet has no shortage of slow people
