It's all about context.
Historically, calling someone gay was used as an insult as it was an attack on someone's "manliness".
These days it's becoming less and less offensive as homosexuality has become more acceptable.
It's likely calling a guy "girly". While still somewhat of an insult to call a man this. Sexism is also in decline so is also less of an insult.
