I frequently wear a neoprene cockring at the gym but I put it on discreetly before going out on the floor and remove it the same way before stripping down to change for a shower or the pool/sauna areas. I have seen guys with them still on in the locker room before but these have mainly been at gyms with a large gay clientele. I always go commando on the gym floor and the cock ring puts my fat package in even better view under my loose gym shorts. I recommend neoprene over metal as they are easier to take on and off and you are less likely to have it fall off while walking around the gym floor.

A metal ring worn behind the balls is unlikely to fall off unless it is way too big or the wearer's balls are tiny.

Elastic rings of any material are dangerous in that they apply constant pressure to important arteries and veins.

A properly sized metal ring is actually far safer.

And they look nicer, too.

I've worn one for years at the gym. I mean, I wear one most of the time, but I have some silicone ones I prefer for workouts. I'm a jockstrap guy and I find the combination keeps the boys out front and out of the way.

As for it being a big deal I've never noticed it to be a concern unless it attracts to wrong kind of attention. Its important to weigh your surroundings. In gay and commercial gyms, its been no big deal. A lot of guys will wear them. At the downtown Y were its mostly old men in the mornings, I might make some small effort to conceal it. Those times I usually wear a clear silicone one thats almost invisible in the showers and steam room.
