Key word there is HYPOGONADAL. Those people likely had low testosterone for years, but now with therapy, their erections are just regaining their previous level of hardness.
Think of a trans man. Many on hormone therapy for masculinization have large clits, but there are generally not more than 2-3 inches hard.
There are feedback mechanisms when it comes to things like this. For example, testosterone makes you grow tall - but it also causes your growth plates to close- boys who have precocious puberty grow really really tall for a 6-year-old, but as adults, end up shorter than average because the growth plates in their long bones close too early.
It is very unlikely that just smearing testosterone foam on your wiener will do anything other than severely mess you up.
