He is American/Canadian and his parents were there abroad for work. Italy doesn't have birthright citizenship. He still might be circumcised. If it is a cultural practice or expectation it is generally easy for expats to get it done abroad. But maybe!

His Homeland is still Italy so I doubt he's gotten it done as a European

Again, Italy doesn’t have birthright citizenship. How do you know his homeland is still Italy and that he has a European passport?

I think the general point is that based on his parents' jobs (his father was a dean of students at Loyola University's Rome campus), looks like the McHales were "expats" rather than immigrants (ie. they were North Americans living abroad for a temporary stint, keeping up the customs of their country, probably living in community with other expats/North Americans affiliated with the University and not north americans looking to integrate into Italian culture/customs). In fact, it seems like Joel didn't even spend any substantial time in Italy, growing up in Washington. I'm inclined to think they would have him circumcised in keeping with US custom at the time even if they were abroad. But there is of course no hard evidence either way.
