The context is this thread, and other threads on here. And no, it is not purely universal. I have lived in many countries as a gay man, and Americans are extra obsessed with size and if you are cut or uncut in an almost neurotic and hysterical way (and some of this has reached other countries through the dominance of US media, but not to the same extent). I think it is because of how puritanical the US view on sex and bodies are, as well as the idealization of bodies that permeates the culture. You can see it clearly in the difference between how bodies are portrayed in moves, for example. European actors generally don't starve their bodies and/or go to the gym every day for a role.
