From what i see, there’s a watermark that scrolls by a bit. It says that the video belongs to Xhamster, which sounds right, since this is more like one of those fake medical exam fantasies one would think of. Even the sounding portion got me.

I’m curious if anyone is willing to find the original video with sound on Xhamster?

I don't know about this vid, but the other vid they posted was from huzzbears

He's actually testing a reflex. He's scraping the key along the side of the thigh on each side, and the corresponding testicle rises. Fun fact, if you squeeze the head of a boner, the balls should do the same thing.

It's called the cremasteric reflex that he's testing. I had it done when I went to my PCP to deal with scrotal pain. This reflex helps in evaluation of acute scrotal pain and assessing for testicular torsion (associated with loss of reflex). Much like the man in the video, I got naked from the waist down and lay on the exam table. The dr. took a pen and ran it lightly up both inner thighs to check if the reflex was there and if he could visibly see the testicle move. They did, so the nerves were okay. It's a little like when the dr taps your knee with the rubber hammer to see if the leg reacts. Then, he did the examination of the other privates, which I had had done many times, but it was the first time for the cremaster. Since nothing acute was found, I was referred to the urologist.
