Lots of celebrities use their voice to express their own struggles and as I recall he mentioned something about this on an interview after Baywatch on Ellen. He specifically mentioned that it was unreasonable for him, or anyone else, to expect to maintain that type of physique. He also has talked publicly about his struggles in his late teens.

I was asking a question and even apologized for it. So calm down man. lol

I don't think he have body dismorphia, look at his body in this Down to Earth series (filmed after Baywatch)

he probably wants to show he take his work seriously (even if "his work" means to get in shape for a role)

Hollywood is a very competitive place

Personally I think the excessive exercise and possibly human growth hormone have more to do with the changes in his face than anything else.
