Notice how very small framed and thin that woman body and shoulders are, it is highly likely she has small hands as well. Therefore I would say using her hands to estimate his length in that case is not that accurate even more so than normally in this case.
I just noticed her face is also thin and narrow, she has a small frame all the way round like I said these factors make using that photo for reference misleading.

I think your overthinking this stuff dude, and I don't know what porn your watching but 99% of every porn video that I've ever watched included orgrasms in the scenes.
