you can buy an attachment to use with a regular pump, if you want to use water, just stops water from going back into the actual pump

You need to keep water out of your air pump if you are using a pistol style pump with an internal spring which would rust out, a simple bleeder valve canister in-line works, Harbor Freight cheap

I pump in the hot tub regularly. I just unhook the pump from the hose and use my mouth to draw vac. This pic is my tube in water

Does that have any affect on your sensitivity? I am scared to try it but I want to have a good size too look at when I have clothes on. Please advise

I haven't had much issues but after the session when it is still fully pumped it is less sensitive but if your do it slowly there shouldn't be a huge issue. But with any enlargement there is risk of some sensitivity loss if done too quickly or too extremely.
